Position: Support Workers (supported living)

Location: Cerrig Cornel - Tywyn - Gwynedd

Hours: Full time, part time and bank positions available

Salary: £7.90 + benefits

No day is ever the same and every day is a whirl of activity.  The people we support are always out and about doing various activities on a daily basis. If you are a lover of the outdoors you will fit in nicely and if you are a bit of a pro at pool even better! Any budding artists and music lovers are welcome too as the people we support enjoy a variety of activities. The most important thing is that you are passionate about what you do and compassionate towards the people we support.

About Us:

Cerrig Cornel is a traditional 19th century Welsh farmstead that has been converted to provide a range of separate households with modern interiors. Service Users hold their own tenancies at Cerrig Cornel and are supported to maintain these in preparation for eventual step-down into the community if and when that is considered appropriate. Cerrig Cornel is situated with stunning views of the Dysynni Valley and a rural setting which offers peace and tranquility. Cornel is however just a short bus ride away from to local amenities such as a train station, leisure centre with pool, beach, cinema, medical centre and shops. The service is set up for a younger adult service user profile and employs the active model of support ensuring each individual has access to outcome-based activities and has an on-site activities coordinator, so each person will have an individual profile of activities and opportunities. The service is registered to provide support to people who have learning disabilities, including those with Autism/Asperger syndrome.

Role Overview – Support Worker 

You will provide care and support personalised to the individuals we support in line with their Care Plan. You will work within the team to provide individual high-quality person-centred care to individuals we support, supporting each to reach their optimum potential.

The ability to drive and access to your own vehicle would be desirable, but it is not essential.

 Essential – Support Worker 

  • Willingness to achieve (training) in line with regulations
  • Good basic numeracy & literacy
  • Proficient in IT; Word, Outlook, Internet
  • Participation in a wide range of Service User activities in the Service and community
  • Willingness to provide a range of care, relating to individual care plans including personal care
  • Ability to work within policy, process and systems of work e.g. medication management

Principal Tasks - Support Worker

  • Engages with and follows their Care Plans
  • Updates their Care Plans and risk assessments
  • Uses company processes to capture information and maintain individuals we support files
  • Undertakes identified goal-oriented activities and care for each individual
  • Offers and presents options to support and empower choice and independence
  • Responsible for the capture of outcome information and data, on company documentation and processes
  • Completes specific, delegated tasks to deadlines and quality standards
  • Provides key worker support
  • Act as advocate for the people we support
  • Works in partnership with colleagues, appropriate family, friends, others significant to the individual and care professionals
  • Ensure individuals are safeguarded by working within internal policies,
