Description

Support Worker Required

The service is a Supported Living 6 bedded provision for adults with LD, Acquired Brain Injury, and Mental Health Diagnosis. Ages 25 – 55.

The service is based in a busy and vibrant part of the North West London area of NW10.

We are close to local amenities and have good transport links via Willesden Junction Station, and various local bus routes.

We have a 7 day rota with shifts from) 7.30am- 15.00pm & 14.30pm – 10.00pm, with some mid shifts included. We have a sleep in shift on the rota for all staff, supported by an on call system.

The residents currently are very diverse in their needs and the support they require.

Essentially we are very holistic in our approach, and are guided by the needs & wants of our residents.

Residents are encouraged and supported to fully engage in Community Services and Activities including health, leisure, education, vocational, self-help groups, and community social groups such as MEN CAP.

The successful applicant will be joining a skilled, experienced, and supportive team with extensive experience of the current residents and client group.

The successful applicant will need to complete our E, learning training prior to commencing employment. And will be offered classroom based training essential to statutory requirement, in addition to practice based learning and development opportunities.

Essential – Support Worker

  • Willingness to achieve (training) in line with regulations
  • Good basic numeracy & literacy
  • Proficient in IT; Word, Outlook, Internet
  • Participation in a wide range of Service User activities in the Service and community
  • Willingness to provide a range of care, relating to individual care plans including personal care
  • Ability to work within policy, process and systems of work e.g. medication management
  • Professional manner and presentation
  • Good spoken and written English ability to record information accurately
  • Flexibility to work irregular hours e.g. early morning, evening, weekends, sleep-ins

Principal Tasks - Support Worker

  • Engages with and follows individual Service User Care Plans
  • Updates Service User Care Plans and risk assessments
  • Uses company processes to capture information and maintain Service User files
  • Undertakes identified goal-oriented activities and care for each Service User
  • Offers and presents options to support and empower Service User choice
  • Responsible for the capture of outcome information and data, on company documentation and processes
  • Completes specific, delegated tasks to deadlines and quality standards
  • Provides key worker support
